Our clinic offers Immigration exams for obtaining green cards through the USCIS. Immigration exams consist of a lab-only visit for bloodwork and vaccines, and a visit with the physician for the medical examination and completion of the I-693 form approximately one week later. Insurance companies do not cover the examination however, vaccines can be billed through insurance. If you have all your vaccine records up to date and in English, we can expedite the medical examination. We can also see you for an initial telemedicine consultation if evenings work better. Be sure to state that on the form if an initial after hours telemedicine consultation is preferred, please note that the fee will be $510 if you choose to do an initial telemedicine appointment to expedite the process.
For your Immigration exam, you will
need to bring the following things:
-Picture ID
-Insurance card
-Immunization records (if you have any)
- Printed and filled copy of the I-693 (https://www.uscis.gov/i-693)
Basic medical examination, Tuberculosis, gonorrhea and syphilis testing, completion of I-693: $510 plus tax.
( Note that if your Tuberculosis Test is positive, you need a chest x-ray. which can be done at our location.

TDaP
COVID
Varicella (if no history of previous infection)
Influenza (Seasonal)
