Minneapolis Health Clinic offers USCIS immigration medical exams (Form I-693) for people applying for a green card or adjusting their immigration status. Dr. Grace Totoe is a USCIS-designated civil surgeon. The published basic civil surgeon fee is $650 plus tax and includes a medical history review, physical exam, tuberculosis screening, syphilis and gonorrhea testing, and completion of Form I-693. Vaccines are billed separately, and a chest X-ray may require an additional charge if a tuberculosis test is positive. Same-day appointments and walk-in availability are offered.

What Does the Immigration Medical Exam Include?

Your immigration medical exam at Minneapolis Health Clinic includes a comprehensive medical history review, a thorough physical exam, tuberculosis screening, and blood testing for syphilis and gonorrhea. Our civil surgeon, Dr. Grace Totoe, will also review your vaccination records to ensure you meet USCIS immunization requirements. Common required vaccines include MMR (measles, mumps, rubella), hepatitis A and B, TDaP (tetanus, diphtheria, pertussis), polio, varicella (chickenpox), and the seasonal influenza vaccine. COVID-19 vaccination is no longer required by USCIS for adjustment of status applicants as of January 2025.

How Much Does the Immigration Medical Exam Cost at Minneapolis Health Clinic?

We offer a flat fee of $650 for the basic immigration medical examination, which includes the medical history review, physical exam, tuberculosis screening, syphilis and gonorrhea testing, and completion of your Form I-693. Please note that if your TB test comes back positive, a chest X-ray will be needed and may incur an additional charge. Vaccines that you may be missing are billed separately and may be covered by your insurance. 

What Should I Bring to My Immigration Medical Exam?

To make your immigration medical exam as smooth as possible, please bring the following items to your appointment: a valid government-issued picture ID (such as a drivers license or passport), your insurance card if you have one, any available immunization records from your home country or previous healthcare providers, and a printed copy of the blank Form I-693. If you do not have your immunization records, we can perform blood titer tests to check your immunity levels for most vaccine-preventable diseases.

For your Immigration exam, you will 

need to bring the following things:

-Picture ID

-Insurance card

-Immunization records (if you have any)

-Printed  and filled copy of the I-693 (https://www.uscis.gov/i-693)

Basic medical examination, Tuberculosis, gonorrhea and syphilis testing, completion of I-693: $650 plus tax. 

(Note that if your Tuberculosis Test is positive, you need a chest x-ray. which can be done at our location.)
